Your 2008 W-2 is available to reprint online
as of February 8, 2009

Please go to www.eprintview.com and follow these instructions:

Enter your Login ID (h followed by your associate ID#)

Enter your Password.

• If you have not logged into ePrintview in the past and changed your password, your password will be the last 6 digits of your social security number.
• If you have forgotten your password, you can click on the Forgot Password? Enter your Login ID, the Employer PIN 0450, and your email address. Your password will be sent to you.

Enter Employer Pin "0450"

Click "Log In " button

You will be prompted to change your password if you have not already changed it. To Change your password:

• Enter your current password (last 6 digits of your social security number)
• Enter new password – must be 6-20 characters, a combination of letters and numbers
• Confirm password by entering it again
• Enter Primary e-mail address
• Click "Change Password"

Click "W-2 Reprint" link

At the "Process W-2 Reprint" page select W-2 form year, 2008 is the current year

Update any address changes if necessary

Select a reason for the W-2 reissue

If you selected the "Continue to print" option, you will arrive at the "Print W-2 Reprint" page. Carefully read all instructions then select the link "Click Here for your 2008 W-2 Form" to view and print your W-2 in Acrobat Reader.

When viewing your W-2 form select the Print Icon on the Acrobat Reader’s menu bar. DO NOT select the File Print option from your Web Browser.

If you selected the "Email" option, you will arrive at the "Email W-2 Reprint" page. Enter your email address and click on "E-Mail W-2" button.

Shortly after you will receive an email from JAT Software with your W-2 form attached. Carefully follow all instructions in the email.
